REGISTRATION:
Early Registration by 5pm est. October 1 - $895
Registration by 5pm est. October 15 - $995
Registration thereafter - $1,195

PAYMENT:
Payment can be made by credit card on this site or by mailing in a check.
Payment in full due prior to event. Payment includes event,program materials,meals, breaks, parking and first night event.

BY CREDIT CARD:
Payment made by credit card via this website will show a charge from Angelfish Meetings & Events on your credit card statement. Please also note an invoice will be automatically mailed to you once the charge is processed.

BY CHECK:
If you wish to pay by check, please make check payable to Georgia State University and mail to CIPI, c/o Angelfish Meetings & Events, 616 Linwood Ave, Ste. A, Atlanta, GA 30306.

Payments made by check are due in our office no later than October 17, 2008.

CANCELLATION/SUBSTITUTION:
Substitution of participants is permissible with prior written notification sent to anna@angelfishmeetings.com. Substitutions can only be made by October 24th.

No refunds unless event is canceled. If a refund is necessary, only ticket value will be refunded.

We reserve the right to change or cancel event. No liability assumed for change in speaker,venue or dates.
